Thesis/Dissertation Forms and New Routing Process (7.25.22)
There has been significant confusion around the forms required by the College for processing theses and dissertations. Working with IT@Sam, Jorge, Breanna, and I have developed a new process that automates the routing of the forms, and we are hopeful that this will make it a more efficient and less painful process for all of us. Before we get into that, I would first like to clarify what forms you need at what point. I am attaching copies of each of them within this page. These forms are separate from the route sheet required by The Graduate School, which you can find here:

Thesis Proposal:
- Appointment of Thesis Committee Form (signed by DCT, Chair, and Dean)
- Thesis Prospectus Form (signed by committee members, DCT, Chair, and Dean)
- Please note that a prospectus (short form of your thesis proposal) needs to be attached with this form.
Thesis Defense:
- Thesis Defense Form (signed by committee members, DCT, Chair, and Dean)
- Please note that an approved, final copy of your thesis needs to be attached to this form.
- Please also note that a grade (pass/fail) needs to be recorded on this form.
Dissertation Proposal:
- Appointment of Dissertation Committee Form (signed by DCT, Chair, and Dean)
- This is the same form as the thesis form described above.
- If you have a committee member who is not a graduate faculty member at SHSU, you must also submit a Non-SHSU Employee Form and receive approval from the College. This form, along with a copy of the outside committee member’s CV, should be sent to the College (Dean Crosby) prior to your defense.
- Dissertation Prospectus Form (signed by committee members, DCT, Chair, and Dean)
- This is the same form as the thesis form described above. Please note that you must also include a dissertation prospectus with this form.
Dissertation Defense:
- Dissertation Defense Form (signed by committee members, DCT, Chair, and Dean)
- These are the same form as the thesis form described above. Please note that an approved, final copy of your dissertation needs to be attached to this form.
- Please also note that a grade (pass/fail) needs to be recorded on this form.
New process:
Breanna, Jorge, and I have been working with Chester Brammel at IT@Sam and have set up an automated routing process, which we think will be a significant improvement over the previous process. Please see below for details on how to initiate the processing of each of the forms covered previously.
- When you settle on a committee, contact Breanna, and Breanna will send you a copy of the committee appointment form and the Non-SHSU employee form, if necessary. Please let Breanna know if you have a committee member who does not work at SHSU so that she can provide you with the Non-SHSU employee form. Once you have completed your end of these forms, send to Breanna, along with a CV of a Non-SHSU committee member and their email address.
- When you propose your thesis/dissertation, contact Breanna and cc your thesis/dissertation chair as verification that you passed your proposal defense. Please include a copy of your thesis/dissertation prospectus. Breanna will then start the routing process for signatures.
- When you defend your thesis/dissertation, contact Breanna and cc your thesis/dissertation chair as verification. Breanna will send you a link to a Dissertation Application web form. Once Breanna receives this form from you, she will start the routing process.
