CARE Network and Absence Notifications

How Care Works
- Step 1: Submit a CARE Network referral (button above) when you or someone you know needs support.*
- Step 2: The CARE Network receives and reviews the referral followed by direct contact with the student.
- Step 3: A meeting is offered for a conversation to explore options
*Note: If someone poses an immediate threat to themselves or others or emergency assistance is needed, please contact University Police at 936-294-1800 or dial 911.
If your matter is regarding any form of discrimination or sexual misconduct, please report the matter to SHSU's Office of Title IX and Dispute Resolution.

Absence Notification Request Form
Class Attendance Policy: Per the Academic Affairs Class Attendance Policy (800401) students may submit an absence notification request as outlined below:
Unscheduled Absence: Unscheduled absences involve unforeseen circumstances such as illness, injury, or severe weather. Instructors may require students to provide documentation to verify the student’s absence. Instructors may not request protected health information from anyone other than the student unless the student has specifically granted the instructor permission to do so. Students may not be required to provide detailed medical information.
Student Hospitalizations / Medical Emergencies: Students may submit the Absence Notification Request Form, with required, supporting documentation, through the Dean of Students’ Office at the earliest possible date. The Dean of Students’ Office will provide notification to the student’s instructors to include the dates and type of absence, as well as an indication of receipt of appropriate, supporting documentation.
For those absences not defined as excused within this policy, instructors may excuse such an absence at their discretion. To the extent possible, students should directly notify course instructors of their absence at their earliest availability.
